Topic information: The Sustainable Model of Early Intervention and Telerehabilitation for Children With CP (SMART CP) was tested in a cluster randomized trial in rural Bangladesh. The model integrated referral networks, caregiver peer groups, community-based centres, and telerehabilitation. Among 968 children with CP, service uptake rose from 70% to 99% in the intervention arm, compared with minimal change in controls. Children in the intervention group accessed rehabilitation earlier, had more therapy sessions, greater assistive device use, and reduced out-of-pocket costs. While no clinical outcome differences were observed, SMART-CP demonstrated effectiveness in expanding access to early CP care and offers potential for global scale-up.
